lack of fusion is just what it says, running too cold or too quick will create a lack of fusion, basically putting down a weld that adheres to on side/or no sides thus lack of fusion.
slag inclusions is a primary visual failure on welds that exhibit holes between passes, either in the field, or on the edges of the weldment. both of these are signs of an inexperienced welder, but don't worry there are plenty of on the job/classroom opportunities for you to become an excellent welder!
